If you are submitting your application via this on-line form, you will be required to provide payment to Chili Soccer by May 1, 2008. Please make checks payable to the Chili Soccer Association.

Certified rosters and medical release forms can be mailed with payment, or you can provide this documentation at tournament check-in.
